Who do I call with any questions?
Please call our Provider Support Team at: (888) 520-5400 or (520) 575-8623, or email us at info@jorgensenbrooks.com.
What is the process for becoming a Jorgensen Brooks provider?
Prospective providers can download and submit the Provider application packet, or contact our Provider Support Team at (888) 520-5400 to request an application package.
How does Jorgensen Brooks Group support its providers?
- Jorgensen Brooks Group does not require authorization for visits.
For example, if an employee has six (6) EAP sessions available, then the provider determines the appropriate course of care. For management referrals or unusual cases, we ask that the provider call us prior to the first visit.
- Minimal provider-friendly paperwork.
- Clinical support. We encourage providers to contact our EAP Director with any questions or for clinical consultation on any individual cases.
- Friendly and professional communication. We believe in working in partnership with our providers to delivery the highest quality care. We provide monthly provider e-newsletters and respond immediately by telephone to any clinical or administrative questions or concerns.
What happens when a client exhausts the number of visits available under the EAP plan?
This question separates the true EAP service from other behavioral health providers. If long term care is indicated, it is the EAP counselor’s responsibility to identify this need BEFORE visits are exhausted, and make the appropriate arrangements for referral to long term care. Self-referrals are appropriate under certain circumstances, but require approval by Jorgensen Brooks EAP. Depending on the case, exhausting services and continuing to see a client while billing the client’s health benefit has been viewed as a potential conflict of interest under EAPA and EACC standards.
Are EAP services kept confidential from the client’s employer?
Yes. The client’s relationship with you and Jorgensen Brooks EAP is confidential. Confidentiality is protected and no information is provided to the employer or any party without the client’s written authorization.
All EAP/Outpatient providers are required to adhere to Federal and State regulations and laws that pertain to the maintenance of confidential information/records. Anyone who divulges or releases confidential information or records concerning any client without proper authorization in accordance with Jorgensen Brooks Group policies, state and federal law may be terminated from the provider network.
